Information Packet for New School Year

Dear Salem Families,

This is your information packet for the 2018-2019 school year. Below are the forms which need to be completed before the beginning of the new school year. You may print and complete these forms and hand them in at Visit the Teacher Night or Welcome Night. We will have these forms available at these events if you prefer the option of completing them the night of your visit. Completing them before you come will make registration much easier and take less time during your visit. The following forms are required for all students (PK-8): 
Required for K - 8 Only:

New Preschoolers and students entering Kindergarten and Grade 7 need to have their school health examination forms completed before school begins. Please have the most current health record sent to our school or contact us if a health form is needed. Seventh graders are required to have the HEP B vaccine. Your clinic can fax health records to 651-439-0035. Preschoolers that have a record on file do not need to send any records if their health record has not changed.

The daily school schedule is 9:15 AM- 3:40 PM. If you live in the Stillwater School District (834) and will be using bus services, you will need to register your child for transportation. Please see the registration form below. If you have any additional questions on transportation, please call the Stillwater Transportation Department for District 834 (651-351-8377).
Here are Important Dates to Note:

Preschool Families - Wednesday, August 15, Visit with the Teachers. Two presentation sessions are available. Come at either 4:00 pm or 6:00 pm. Plan to come for a presentation from Mrs. Grobe on all of the policies and procedures for the school year along with an introduction of our new staff. There will be time to meet your child's teacher and ask questions.

K-8 Families - Wednesday, August 15, 4:00-8:00 PM - Visit with the Teachers.  Each teacher will be stationed at a table in the Fellowship Hall.  Please contact the school office if you would prefer a home visit.

PK-8 Families - Monday, August 20, 5:30-7:00 PM - Welcome Day
Parents and students should come to school to drop off supplies, verify tuition payment, set-up their lunch account, find their desks, and drop off any unfinished paperwork.
